I once saw a horse's past performances where the sire was listed as "Unknown sire" - in that instance the horses mare turned up pregnant unexpectedly and they kept the foal - and it happened to race and I believe win a few times.
Never seen two sires listed in a form before though.

Now they have better testing so they know who the sire is when the foal is born.
There is an old book about Hamburg Place that was stolen from me that was pretty interesting. Seeing Hamburg's name reminds me of how mad I am about that!
