Maybe there are superelite doctors and hospitals that are 100% insurance company or private pay, with no other payment plans, you can go to so you don't catch cooties waiting in line with other patients.
Right now I share doctors and hospitals with other people who have no insurance at all, so I'll be happy when they can finally afford to purchase insurance themselves (with some help) - it will lower my costs.
And I really don't care how other people pay their bills. I mostly worry about mine.
