Steve Jobs - preventable death
 
It's coming out into the news, with the release of Jobs authorized biography:

When Steve Jobs was first diagnosed with pancreatic cancer, it was found early, and was categorized as easily curable with a less than 5% death rate. They recommended excisional surgery.

Jobs declined, in favor of spiritual advisors, diet changes, etc.

When the untreated cancer spread, Jobs then agreed to surgery, but by then it was too late. His biographer says he regretted that decision in the end.
